Our Position on Personal Health Information
At PowerHouse Massage Therapy we are committed to collecting and retaining your personal healthcare information in a secured and respected manner. Information about your health care is obtained to better deliver health care services to you, the patient. This document is intended to outline the procedures and policies the clinic upholds in order to maintain the highest degree of security with your personal health information.
What is Personal Information?
Personal Characteristics · This includes items such as age, gender, telephone number and home address.
Patient Health · This includes items such as healthy history, services received, and other health care practitioners currently treating you. All personal information is kept confidential to the clinic; we do not sell or trade personal data with any third parties.
About Our Clinic
The only person who will have full access to your personal information is Aaron DeBathe RMT/ Clinic Owner. We do use a number of different online services if you are utilizing direct billing. These companies only have access to your name (presented as an account number. Example: #4251, or #1042) and all payment information relating to that account.
Why Do we Collect Personal Health Information?
About Patients · Information such as the name, health history, activities of daily life and all other relevant medical information is collected to better understand and provide safe and effective health care. Your consent is obtained for the collection, use and disclosure of all personal information.
About Members of the General Public · Information may be obtained from the general public such as name, mailing address, e-mail address for the distribution of newsletters, promotions or marketing interests. Your expressed consent is required in order to be included in these types of services. Marketing Information such as name, address or email address may be collected for marketing purposes. This includes newsletters and promotions. Your expressed consent is required for this type of situation.
Billing · Information such as name, home address, postal code, and credit card number may be collected for the purpose of billing. This information is collected In order to provide our patients with a variety of methods in which to pay for services. As the patient you may choose which method of payment you would prefer.
How Information is Protected
All physical information collected, like the patient’s health history, billing information, treatment file and any other relevant documentation will be kept in a locked file folder. All electronic information is stored in a password protected folder on a password protected computer. These account files are kept secure at all times and are backed up regularly onto a password protected portable hard drive.
Retention and Destruction of Personal Information
For Massage Therapy services, records are kept for a minimum of 10 years after the last visit or after the patient reaches, or would have reached, the age of 18. When files are destroyed this means that the complete account content is deleted from the computer and backup drives.
Viewing and Changing Personal Information
You are able to access your personal information. In order to access your information we must receive in writing your request to do so. We will also require you to present two types of identification, one of which must be photo identification. Both pieces of identification must be government issued (driver’s license, health card etc.) The clinic will have a maximum of 30 days to provide access to the file. We will assist you by helping you to understand your file due to the use of abbreviations or technical language. We charge a standard fee of 35$ in order to assist with the cost of retrieval and the time involved in making the file more readable by non-medical personal. If you think you may have a mistake in your file you have the right to request that information be changed. This request must be provided in writing.
